Case Management Nurse

Nursing case management is a complex role in which a nurse collaborates with other health professionals and the patient to coordinate services and resources to assist the patient in meeting their needs. The plan evolves and changes with the patient, and the nurse case management ensures the plan is implemented and helps a patient organize medical appointments and other care.

A nurse case manager typically works with patients requiring long-term care, such as those with cancer, diabetes, brain injury, physical disabilities, or mental impairments, to name a few of those who need home support and assistance transitioning into facility care. The nurse case manager knows community access to supports and resources and has a significant role in patient education around their disease process or injury.

No certification is required to specialize in nurse case management. However, the American Nurses Association does provide board certification for this specialty.
